Treatment options for partial sacralization (Berrolotti Syndrome)There are several treatment options for partial sacralization, or the Bertolotti Syndrome, which go from the hands in the mobilization of pseudarthrosis to surgical resection (osteotomy) of the over-dimensioned transverse L5 process. As the intervertebral disk at the L5 level loses height the 'propping' trauma of the cross-sectional process against the pelvis side increases. Although the degeneration of the disc is unlikely to be painful, the mobilization of the L5 vertebrae centrally (PAP expressions) helps the disk absorb the water, elevates the L5 roof centrally outside the sacral and thus decreases the lateral pressure of the cross-sectional process on the pelvis. With Bertolotti syndrome, mobilization pressures have a double focus: centrally in the spinal process L5 and generally in pseudarthrosis to make this development joint work better. The treatment ideally involves the manual mobilization of false joint with the hands (or in my case, heel) but it is easy to 'overdo' treatment here. Note that pseudarthrosis has developed as a functional necessity, but it is far from an ideal joint. Although the difficult pain point craves pressure, it can be overwhelmed and inflated by too much perforation and stretching treatment. The smooth progressive mobilisation administered at the right speed is spectacularly effective when combined with the right own treatment (see below). Self-mobilization Treatment of SacralizationMobilization Treatment of Sacralization You will see how you can perfectly mobilize a facet joint yourself using a soft squashy mass directly below the joint. A couple of kids' gloves is ideal for this but a tennis ball is too much. The gloves bent back on themselves create the perfect tool, with the leather surface of the sweater creating drag against the skin that keeps the gloves in place. You can do the same to mobilize a pseudarthrosis of Bertolotti's syndrome, taking care to place the glove mass locally, just below the false joint. With the mass of gloves in place you perform small movements of bending-extension of the lower back and pelvis that open and close the false joint. The soft and scaly mass of the gloves loves the soft tissues surrounding the joint, dispelling toxic inflammatory products and helping to make them more compatible. With each session you need perseverance to get gloves in the right place and well-directed pressure. Watch the video for the real time application of this exercise. Auto-Treatment of Sacralisation with Decompression Exercises Auto-Treatment of Sacralisation with Decompression Exercises All the usual exercises of pacification, self-mobilization and strengthening described in can be put into action here. Spinal decompression using a BackBlock is an important part of the treatment as this decompresses pseudarthrosis along with intervertebral disks in L5 and L4. Using BackBlock is an important part of self-treatment for any back problem. Pressure changes induced by the end-of-range movement of the lumbar spine segments act as a physical bomb. The concertine-action local pumping causes a better exchange of fluids both through the intervertebral disk and pseudarthrosis and helps dissipate the inflammatory products of both structures. The improved flow of fluid inside and outside the lower discs also helps stimulate the proteoglycans synthesis, which improves the disk's ability to retain water. A more voluminous disk filled with water at the affected level reduces the grinding contact of the cross-sectional process against the pelvis. You can read more about this. The generalised column mobilization techniques also point to "normal" vertebral joints, increasing their performance and thus reducing the forces at the problematic L5 level. The "banana removal" is an important lateral section for unilateral sacralization. While the 'swimming of the banana' opens the pseudarthrosis through a section of lateral reflection of the trunk, the 'soil screws' open the joint through a rotating or rotating action. You will see in the short attached video that the enderezing of the important leg on the knee helps to fight the roots of the lower lumbar nerve free of adhesions and thus reduces the chronic cyatic that is often associated with Bertolotti syndrome. Surgical options for Bertolotti syndrome Surgical options for Bertolotti syndrome After using local anesthesia to confirm that the sacrailsation is the seat of pain, the first surgery is usually a steroid injection, possibly followed by cautery from the nerve source to the joint. If the effect is positive but not durable (and assuming that the exhaustive conservative treatment has not been useful) there is the option of resecting the expanded cross-sectional process and pseudarthrosis, or merge it. There are only a few studies to go, but it seems that fusion is marginally more successful than osteotomy (bone removal) although the postoperative recovery period is longer. Bone removal is carried out through the use of a high-speed burr. Note that pseudarthrosis often develops an improved nervous source during the period that has become increasingly swollen.
